Persons who accept to serve as guardians of a student to facilitate their enrollment in the Hidalgo Independent School District must recognize the serious responsibilities they accept upon signing this document. This document requires notarization from a licensed notary public in Hidalgo ISD to be valid.

I, hereby accept and agree to said responsibility over my son/daughter which includes making decisions, examining and receiving information about attendance, placement, discipline, grades, health, transportation (if applicable), and residency for school purposes. I, further, understand that it is my responsability to notify the school when the guardianship relationship ends. I also understand that presenting false information is an offense under Section 37.10. Penal Code and that the enrollment of a child under false documents subjects me to liability for tuition or costs.
