Medication Authorization Form - Prescribed or control medications
Parents / guardians if the student's doctor prescribes a control medication that needs to be given during school hours, please have this form be filled out by the doctor, sign by him/her and you then give it to the school nurse or the nurse assistant. Medications given three times a day must be administered by parent/ guardian before school, after school, and at bedtime. Do not send the medication with the student, please give it to the school nurse or nurse assistant. Medications from Mexico will not be administered by Hidalgo ISD personnel. Medications that are not picked up the last school day will be discarded. Padres/tutores si el médico del estudiante prescribe un medicamento de control que debe administrarse durante el horario escolar, por favor que el médico llene este formulario, lo firme él/ella y usted y entrégueselo a la enfermera o asistente de enfermería de la escuela. Los medicamentos administrados tres veces al día deben ser administrados por el padre/tutor antes de la escuela, después de la escuela ya la hora de acostarse. No envíe el medicamento con el estudiante, déselo a la enfermera de la escuela o al asistente de enfermería. Los medicamentos de México no serán administrados por el personal de Hidalgo ISD. Los medicamentos que no se recojan el último día de clases serán desechados.
